Your written diagnostic report is the formal output of your assessment. It is a medical document used to inform your treatment, support reasonable adjustments at work or in education, and communicate with your GP.

Standard sections

  • Demographic and referral details.
  • Presenting concerns and reasons for assessment.
  • Developmental, medical, mental health, and family history.
  • Findings from validated tools used during your assessment.
  • Diagnostic outcome and clinical reasoning.
  • Treatment recommendations and practical next steps.
  • Signposting to additional support.

How to use it

Keep a digital copy and a printed copy. Share with your GP if you wish to enable shared care or to add the diagnosis to your medical record. Share with employers, universities, or schools to support reasonable adjustments. You control who sees it.
