EHCP, SEND and your assessment report
An Education, Health and Care Plan is a legal document that secures additional support for children and young people up to the age of twenty-five who have special educational needs and disabilities. A diagnostic report can play an important role in this process.
What an EHCP can include

Who applies
Parents, carers, the young person themselves from age sixteen, or the school can request an EHCP needs assessment from the local authority. The local authority decides whether to assess and, if so, whether to issue a plan.
