Pre-assessment questionnaires explained
The pre-assessment questionnaires are not a test. They are structured rating scales that help your clinician arrive at the appointment with a clear picture of your history and current functioning.
What they cover
- Childhood symptoms across attention, activity, and impulsivity.
- Current symptoms across the same areas.
- Impact on home, work or education, relationships, and finances.
- Mental health history, including any current diagnoses or medication.
- Family history of ADHD, autism, and related conditions.
How to approach them
Take around forty-five minutes in a quiet moment. There are no right answers. Honest, considered responses are more useful than rapid-fire ones. If you are unsure about a question, mark your best estimate and we will discuss it during the appointment.
If you are completing them for your child
Use your knowledge of your child across multiple settings: home, school, social. Where you have school reports, end-of-year comments, or specialist letters, you can refer to them as you complete the questionnaires.
What happens to your responses
Your responses are stored securely in compliance with UK GDPR and form part of your clinical record. Only your clinician and authorised members of the Lighthouse Pathways team have access.
